Prepare the following documents:

Family Information Sheet

On a handwritten sheet, include the following information:

1. Applicant’s full name, age, date of birth, current school, previous and current school average grades, and home address.
2. Mother’s name, education level, age, home address, and occupation. If employed, explain where she works and her work schedule.
3. Father’s name, education level, age, home address, and occupation. If employed, explain where he works and his work schedule.
4. List all people living in the same household and sharing food expenses: family member’s name, relationship to the applicant, education level, age, and occupation. If they work or receive income or a scholarship, describe the monthly amount contributed to household expenses.

Employment Verification Letter

A letter from the primary provider must be submitted. If both parents work, both must provide letters, even if they do not live together. If you live with relatives who contribute to household expenses, they must also provide employment letters.

1. If employed by a company, request an official letter from Human Resources specifying name, position, schedule, monthly income, length of employment, date, and the name, signature, and contact number of the HR representative. Attach your two most recent pay stubs.
2. If you work for an employer, that person must write a letter including the same information listed above, along with their name, signature, and phone number.
3. If you are self-employed, write a handwritten letter describing your work, where you perform it, your average monthly income, and how long you have been working in that activity. Include your name, signature, and date, as well as the names, signatures, and copies of the IDs of two witnesses who can confirm your occupation.

Housing Letter

Submit a handwritten letter specifying who owns the home, the address, how long you have lived there, whether the home is owned, rented, or borrowed, and the monthly rent and utility expenses.

Describe the characteristics of your home (number of bedrooms, flooring and roofing type, whether spaces are shared with relatives or tenants, etc.).

This letter must include the date, your name and signature, and the homeowner’s name and signature. If you rent, include a copy of the lease agreement.

Copy of Proof of Address

Must be no older than two months.

Vehicle Information

If you own a car or motorcycle, include a copy of the registration card.

If you do not own the vehicle but use it to take your children to school or for work, submit a handwritten letter describing the vehicle (brand, model, license plates, and color), who owns it, what it is used for, the date, your name and signature, and the owner’s name and signature.

Letter of Intent

On a blank handwritten sheet, please share with the Scholarship Committee why you are interested in receiving a scholarship for your child.

If there is any particular situation involving the applicant or the family, you may describe it in this letter.
